Licenses for temporary restaurants can be obtained by downloading the application here. Print the application and submit with the appropriate fee (refer to fee schedule below) to the Josephine County Public Health Department. Applications with payments can also be mailed to the following address:

PLEASE NOTE: Applications that are received 0-7 days prior to the event may have an additional fee charged (refer to fee schedule).
Upon receipt and approval, a licensed Environmental Health Specialist will contact you and/or visit your booth during the event in order to complete a regular health inspection of your facility as well as to sign off on the restaurant license.
