Citizen Complaint Form

As your Sheriff, I am committed to holding Sheriff's Office employees to the highest ethical standards possible.  Consistent with this goal, I have communicated to all employees of this organization that a citizen's report of possible misconduct or error in procedure will be thoroughly and impartially examined by an investigator who reports to me. 

All Sheriff's personnel must deal with citizens in a fair, impartial and courteous manner.  Even in contacts with suspected law breakers, we cannot assume that such a person is guilty of a crime. 

This complaint form does not address complaints originating from an interpretation of the law; issues of this nature are the proper concern of our courts. 

Should you wish to register a complaint about the conduct of a member of the Josephine County Sheriff's Office, or the application of existing policies or procedures we ask that you proceed as follows:

Use the form links below in Word or by Adobe Acrobat.  The forms can be mailed to us at 601 NW 5th Street, Grants Pass, OR  97526 or visit us Monday through Friday from 8:00 a.m. to 5:00 p.m.  Additionally, you may complete this form online by following this link.

You will be asked to provide your name, address, telephone number, names of any witnesses and a description of the incident.

Please be advised that the Sheriff's Office takes all complaints against members of the Office VERY seriously; filing a false, untrue, or exaggerated report may result in legal action.
